Le nécessaire questionnement éthique autour de la relation de soin en télémédecine bucco-dentaire

Abstract

En transformant la relation de soin, la télémédecine, en particulier bucco-dentaire, virtualise la rencontre médicale et produit une mutation du modèle relationnel classique qui réarticule la relation entre le praticien et le patient. Elle introduit un levier d’action positif dont l’utilisation doit rester à l’appréciation de la balance bénéfices/risques que l’éthique devra aider à établir pour chaque patient. La télémédecine doit donc être comprise comme un moyen et non comme une fin, et ne doit pas être fantasmée au point d’en faire un modèle de prise en charge universel. Il semble donc recommandé d’interroger systématiquement les innovations technologiques et le développement de protocoles à l’aide d’une réflexion éthique afin d’évaluer a priori et a posteriori le progrès envisagé.

Technological innovations in the fields of medical imaging and communication have enabled the rise of dental telemedecine. By transforming the usual caring relationship, dental telemedicine virtualizes the medical procedure and generates a mutation of the classical relational model that rearticulates the relationship between the dentist and the patient. It introduces a positive lever that must stay up to the appraisal of the benefit-risk equation, that an ethical questioning will help to evaluate for every patient. Dental telemedicine must be understood as a mean, and not as an end. It must not be fantasized as a universal dental care model. It seems to be recommanded to investigate systematically the technological innovations and the development of protocols thanks to ethical guidelines in order to evaluate a priori and a posteriori any kind of progress that might be considered.
