dc.description.abstractOn suppose que la perte de la fonction de réparation des mis appariements (MMR) dans une cellule normale ne permet pas sa survie : au bout d'un nombre restreint de mitoses, le phénotype mutateur doit entraîner l 'extinction du clone par déclenchement de l'apoptose ou sénescence accélérée. Cette hypothèse conduit à concevoir un processus d'initiation hétérodoxe de la cancérogenèse en plusieurs étapes, menant à une prolifération explosive après une phase préliminaire abortive : la perte de la Jonction MMR ne peut aboutir à un clone malin qu'à condition d 'être rapidement suivie de mutations rendant la cellule tolérante au phénotype mutateur. La probabilité d'acquisition des mutations salvatrices dans les clones mutateurs est Jonction (1) de la tolérance des cellules souches aux mutations ; (2) du nombre de mitoses dans la couche proliférative. Le remplacement du paradigme déclenchement-promotion par ce nouveau concept de cancérogenèse permet de comprendre les énigmes de la relation génotype-phénotype du syndrome de Lynch et ouvre une perspective de chimioprévention des cancers avec instabilité des microsatellites.fr
dc.description.abstractThe genotype-phenotype relationship of Lynch syndrome displays many curious features which cannot be explained by the prevailing concepts of carcinogenesis. In order to explain these anomalies, we propose a model of replication error (RER)-associated carcinogenesis based on the simple idea that many random mutations in normal cells either trigger apoptosis or accelerated senescence, i.e., acquisition of a mutator phenotype almost always gives rise to short-lived clones that vanish without a trace when the mutation load becomes incompatible with life. It follows that RER+ carcinogenesis can start only if several mutations blocking apoptosis and senescence, all acquired during the short life span of a non-transformed mutator clone, eventually rescue one RER+ cell. The rescued mutator cell then unavoidably gives rise to a malignant sub-clone that further progresses at an accelerated pace toward cancer. All the curious features of Lynch syndrome can be explained by concentrating on the physiological time and space variations of the two key parameters which modulate the very low probability of completing the non-orthodox rescue-and-initiation process once a cell has acquired mismatch repair deficiency. These parameters are the capacity of the stem cell to tolerate a high mutation load and the number of mitoses in the proliferating compartment of the tissue.en
