De la neurasthénie d’Adrien Proust au syndrome de fatigue chronique

Depuis son introduction dans les années 1870, jusqu'à sa disparition près d'un siècle plus tard, la neurasthénie a zigzagué entre la neurologie et la psychiatrie, considérée tantôt comme une maladie physique, tantôt comme une maladie mentale. Aujourd’hui la neurasthénie a disparu comme catégorie diagnostique. Ces modifications catégorielles soulèvent la question sur la « réalité » et le type de trouble qu’est/était la neurasthénie et ses formes ultérieures. Dans quelle mesure ces diagnostics correspondent à de « vraies » maladies et à ce que les philosophes appellent a natural kind ? Les diagnostics changent, les cultures médicales et les patients changent à travers eux, transformant l’expérience de la maladie. De telles interactions justifient les regards croisés entre les sciences de la vie et les sciences humaines et sociales, entre médecine et littérature. Dans cet article, l’approche psychiatrique est accompagnée par la présentation du livre L'Hygiène du neurasthénique, qu’Adrien Proust publia quand son fils ainé Marcel avait 26 ans. Marcel se reconnut évidemment dans les traits décrits par son père et les reprit à son compte, créant ainsi un des portraits les plus brillants et plus touchants de la littérature mondiale sur ceux que l’on appelait alors des neurasthéniques. From its introduction in the 1870s until its disappearance almost a century later, neurasthenia zigzagged between neurology and psychiatry, sometimes considered as a physical illness and sometimes as a mental illness. Today neurasthenia is replaced by new diagnoses. These modifications of diagnostic categories raise the question of the "reality" and the type of disorder such as neurasthenia and subsequent related diagnoses, and to what extent these correspond to what philosophers call "a natural kind". Diagnoses change, medical cultures and patients change through them, and in turn change the patients’ experience of illness and disease. It is such interactions that justify and give meaning to the interplay between the life sciences and the human and social sciences, between medicine and literature. In this article, the psychiatric approach is accompanied by the presentation of the book, L’Hygiène du neurasthénique that Adrien Proust published when his eldest son Marcel was 26 years old. Marcel obviously recognized himself in the features described by his father and took them over, thus creating one of the most brilliant and touching portraits of the world literature on persons who were called neurasthenics in the late 19th and up to the middle of the 20th century.
